Our care coordinator is involved in the personalised care of patients.

Focusing on patients with long term conditions who need annual tests and reviews of medications, making sure all relevant tests are done prior to review. Offering vaccinations when eligible and working alongside the digital transformation lead to improve the care and services offered to patients.
