What is a PCN?

PCNs (Primary Care Networks) were developed by the national association of Primary care (NAPC) to strengthen, redesign and improve Primary care.

Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as Diabetes, heart disease, mental health issues hypertension and many more.

Having these additional roles allows us to link staff from general practice, community services, mental health services, social care and voluntary organisations in the local area to deliver Joined up care.

We can now offer a diverse range of services and care for our patients that we could not as single practices.

LMN PCN is made up of three surgeries, Kent House Surgery, New Ash Green surgery and Meopham Medical Centre.
